Host Family Application

To officially apply to become a Global Insights host family, you must complete the Host Family Application. In order to download the application, complete the form online, and save the data to your computer, you must have Acrobat Reader version 8 or higher installed on your computer. (If you do not have Acrobat Reader version 8 or higher, obtain/upgrade this free software by clicking on the “Get Adobe Reader” box below. Then come back to this page.)

IF YOU HAVE TROUBLE OPENING THE FORMS; YOU MAY NEED TO UPGRADE THE ADOBE READER.  Downloading the READER 9 is FREE and easy.  If you have version 8 or higher, you can complete the forms online, save them to your computer if you don't have time to do it all at one sitting, and email the completed forms.   DOWNLOAD ADOBE 9 READER NOW

If you already have the preferred version of Reader installed on your computer, you can open the Host Family Application by clicking on the link below. Use your computer to type your responses directly onto the application or print out a blank form and complete it by hand. In either case, sign and date the completed form and give the original to your local Area Representative or mail it to:

Complete Host Family Application Packet
The complete Host Family Application Package contains the Host Family Application and Agreement, two Criminal Background Check Forms, and two Host Family Photo Pages. Global Insights is required to run a criminal background check on all household members, 18 and over, including those living part-time in the household. You have the option of completing the background check forms contained in this file and giving them to your local Area Representative, thereby authorizing Global Insights to run the background checks. Alternatively, after obtaining a password from your local Area Representative, you may go to our online portal at https://globalinsights.volunteerportal.net/welcome.aspx, enter the pertinent information, and authorize the check yourself. If you choose to use the online portal, you may leave pages 9-10 of this file blank.
 

Extra Criminal Background Check Forms

The complete Host Family Application Packet above contains two Criminal Background Check Forms. If you have more than two persons over 18 in the living full or part-time in home, and opt to complete the paper version of the criminal background check instead of using the online portal noted above, you can download additional Criminal Background Check Forms here.

Online photo submission form
The computer Host Family Application Packet above contains two Host Family Photo Pages which you can print out and paste/tape host family photos to and submit to your local Area Representative. Alternatively, you can use Online Photo Submission Form (with Acrobat Reader 9) to load a photo to the form, type in a caption describing the photo, save the file and then submit the form to our office by email (either using the email button or by saving the form and sending it as an attachment). Please make sure to enter your name, the student ID number and name (if known). Due to the limits on the size of attachments that can be emailed, only one photo is allowed per form. Please re-size your photos to no larger than 4 X 6 prior to submission and feel free to use as many pages as needed.
